Raphel is an important character in Baldur's Gate 3. As a devil, he presents offers that people can't refuse, only to realize that they made a bargain that has to be kept, for good or ill. How to Complete Deal With the Devil in Baldur's Gate 3
The Baldur's Gate 3 Deal With the Devil quest happens during the third act, specifically once you reach the town of Rivington. As you're doing the Open Hand Temple Murders task, you might come across the Sharress Caress pleasure house. You'll find Raphael in one of the rooms on the second floor.
Should You Accept Raphael's Proposal
Raphael proposes the following: he'll give you the Orphic Hammer, which is the only weapon that can free Orpheus. In exchange, you must hand over the Crown of Karsus once you've obtained all the Netherstones. This will allow him to rule the Nine Hells. Here are your choices:
You can accept the offer, in which case you'll be bound via a pact with the devil.
You can reject it outright.
Lastly, you can mull it over (i.e. stay undecided for now). Bear in mind that getting the Crown of Karsus is a late-game objective since you have to acquire all three Netherstones:
The other two are held by Orin the Red and Lord Gortash. You've got quite a few things that you need to do before you can take them out. Surprisingly enough, they're also rivals, which means they're trying to coax you into allying with them, too.
How to get out of your deal
If you accept the Baldur's Gate 3 Deal With the Devil proposal, you're told that you have to keep your promise (i.e. hand over the Crown of Karsus to Raphael later in the game). Going against this pact would mean losing your soul for eternity.
However, there's a hint stating that your contract is kept hidden in Raphael's domain. Perhaps there's an opportunity that allows you to steal it right from under the devil's nose.
As an aside, we went a different route: we did not accept Raphael's proposal, but we still entered his domain. That's where we found the Orphic Hammer on our own. Your only benefit to choosing to accept Harleep's deal (unless your character would enjoy these random encounters) is that Harleep will tell you the location of Raphael's safe and bestow the key to unlock it freely. Choosing to accept Harleep's deal prevents you from completing the Helldusk set.
You can trust Raphael in the short term, sure, but later in the game you'll want to decline his deal made in Act 3, where he offers you a specific weapon, and work to steal it from him instead. He's a devil by nature and by trade, instinctively cunning and controlling.
As the narrator advises, if you wish to get out of the deal, you can go see the Diabolist Helsik at the Devil's Fee in the Lower City and enter the House of Hope to steal back your contract. If you don't do this, you'll get a very specific ending involving Raphael.
Doing so will lead to one of the toughest optional boss fights in the game, but it'll also give players the most options when it comes to endgame choices. If players do want to enter the House of Hope, they should accept the deal.
Accepting his offer leads to disapproval from the core party members, along with major sacrifices in later parts of the game. Rejecting Raphael's deal has few negative consequences. The party can pursue other routes and even accomplish the same result without the cambion's help.
Haarlep will then invite the party member to surrender their body one last time: if rejected, a fight ensues; if accepted, Haarlep will finish and assume the party member's form, adding their likeness to their shapeshifting repertoire.
If you're wondering about the Baldur's Gate 3 Raphael romance, then the answer is "no." To be more specific, you don't actually get to sleep with Raphael himself, but there's an incubus that bears a striking resemblance to him. The incubus' name is Haarlep, and you meet them in the devil's domain in Avernus.
Romancing Haarlep - Your relationship with Gale does not affect this option triggering, nor does going through with it have any implications on your romance with Gale afterward.
Letting Astarion bite you is an easy way to get approval with him. The simple answer is yes, as the only downside is gaining the Bloodless debuff, while Astarion gains the Happy buff and his approval rating of you increases.
While you can refuse, Astarion will be upset, and you'll need to deal with Raphael's nemesis anyway, so it's worth accepting the quest. If you successfully kill Yurgir for Raphael, he'll appear in your camp during your next long rest to explain the marks on Astarion's back.

When Raphael is defeated, Hope is finally free to live her life without the constant threat of torture. While she feels she cannot leave, she is confident that she'll turn the House of Hope into a place that truly lives up to its name. As a reward, you receive the Gloves of Soul Catching.
Raphael can remove the tadpole in your head in an instant, but is this option worth owing him one? You'll run into all manner of characters during your journey in Baldur's Gate 3, but one of the most peculiar is Raphael, who appears behind your party during Act 1.

The immediate consequence of saving Orpheus in Baldur's Gate 3 is that he will join your party as an ally and accompany you the rest of the way through the game and into the final fight. However, the flip side of that consequence is that you will lose the Emperor as an ally. He will completely leave your party.
Intrigued, Haarlep will offer valuable information that will aid the party on one condition: one of them must "play" with Haarlep. If the invitation is accepted, Haarlep will command them to undress and lay on the bed.
